Advocates for Human Potential, Inc. (AHP) and Primary Care Development Corporation (PCDC) are collaborating to support the development and operations of social care networks, using our wealth of industry knowledge, vast experience, and our commitment to helping healthcare providers deliver high-quality care to disinvested communities.

PCDC is a nationally recognized nonprofit organization and U.S. Treasury-certified community development financial institution with 30 years of on-the-ground primary care operations, integration, and workforce development experience in New York State.

For over 35 years, Advocates for Human Potential, Inc. (AHP) has been improving health and human services systems of care and business operations to help organizations and individuals reach their full potential. AHP has extensive experience in criminal justice and justice-involved populations, from conducting assessments to planning programs, and from implementation to evaluation.

Addressing Social Needs, Engaging Health Care Systems

AHP and PCDC operate based on strategies informed, designed, and approved by leading subject matter experts in the intersection of 

  • High-risk populations, such as people with intellectual and developmental disabilities, Black, Indigenous, and people of color, and pregnant and parenting women;  
  • Social drivers of health, such as unstable housing and homelessness, criminal justice system involvement, gender-specific concerns, and issues specific to urban, suburban, and rural populations; and  
  • Mental health and substance use disorders.

AHP Offers:

  • Health-related social needs (HRSN) screening and navigation training for primary care and behavioral health professionals;  
  • Revenue cycle management consultation for organizations screening, referring, and providing HRSN services; and  
  • Quality improvement related to HRSN activities in clinical settings. 

Optimizing Care in Behavioral Health Environments  

AHP and PCDC help new and developing healthcare professionals operate in public health and health-oriented environments by  

  • Informing curriculum design to ensure alignment with requirements of advanced primary care and behavioral health care models;  
  • Enhancing primary care and behavioral health professions with new knowledge, confidence, and skills;  
  • Developing skills around HRSN screening and navigation, Motivational Interviewing, trauma-informed care, and burnout management; and  
  • Developing managers’ skills to be effective in new care models with new professions and roles.
