The Communities We Serve

Homelessness and untreated behavioral health issues are compounded by societal challenges such as the misuse of new technology, volatile economic conditions, and rising crime rates. The good news is, our deep understanding at the intersection of these issues empowers communities to improve health and well-being.

State

We provide services to state agencies across sectors that include health and human services, behavioral health, housing, criminal justice, and Medicaid. These agencies aim to make innovative, positive changes to statewide conditions for providers and the public alike. However, they may lack proven models and the capacity to plan and implement such changes.

That’s why we develop strong relationships with state agencies. These close partnerships aid in identifying challenges, planning solutions, and providing support for managing state distributions of funds to grantees. Through assessing existing programs for making improvements and designing new ones, we can achieve real, lasting changes to the most complex health and human service challenges.

We ensure compliance and accountability for all funding, offering training and technical assistance, conducting research and evaluation, providing consulting, and supplying the data for reporting and storytelling. Federal

We work with federal agencies responsible for policy development, implementation, funding, and performance of America’s health, behavioral health, and human service systems. We serve their needs by providing technical assistance, research and evaluation, and more to help at-risk individuals and families access better care and achieve better health.

Provider

Our experience across federal, state, and county levels benefits providers for their next mission. From new payment models like value-based reimbursement (VBR) to clinical models like care coordination and integration, our local, short-term projects do more than solve system challenges. They leverage the provider community to expand access, diversity, equity, and quality for patients.
